Create two formative assessments. One assessment is for students without disabilities and the other is the SAME assessment that incorporates accommodations and modifications for students with disabilities (must use a minimum of three disabilities identified in the instructions for Part 3 of your final lesson plan – see appendix B). You MAY use these assessments as part of your Part 3 Final Project lesson plan.
Choose at least three of the disabilities:
1 student with mild cognitive challenges
1 student with ADHD
1 hearing impaired student
1 visually impaired student
1 gifted student
Formative assessments can multiple choice. Please let it me for 3rd or 4th Grade using the Mississippi College and Career Readiness Standards

General Education Students
“Rhyming Rainbow”
Directions:
1. Students will cut and paste the pictures by placing them in the correct category by color
on the sorting map.
2. On another sheet of paper, students will then pick two pictures from each category and
draw 5-6 things that rhyme with them. They will label the picture they draw with the
correct rhyming words.

Students with Disabilities Formative assessment
Students with ADHD:
-Pictures will already be cut out for students. Students will paste pictures in the correct category.
On separate sheet of paper, students will only choose one picture from each category and draw
two things that rhymes with the object. Students may also be provided with a Rhyming sheet to
assist them.
Students who are visually impaired:
-Pictures will also be cut out for these students. Each will category will be colored instead of just
having the word of the color. Ex: Blue category will be a blue column. Students will also have
larger pictures with a larger sorting map. The teacher will call out words and ask students if it
rhymes with the picture in the categories. Teacher will check for understanding of the
assignment.
Students with hearing impairments.
-Students will be given assignment in sign language. Students will be able to cut pictures out and
place them in the correct category. Students sorting map will also be colored categorized. They
will also have a list to assist them. Their list will include pictures of words. Teacher will check
for understanding of the assignment.
