Case Study
Tom is 17-year-old young man who has been described as a troubled child since the age of seven or eight. He has a long history of fighting with other students, biting his classmates, and stealing from his peers, parents, and siblings. Tom has been caught shoplifting and was prosecuted on two occasions, one of which resulted in his father paying a large fine and the other resulted in six months of probation.
Tom has held a couple of low-paying jobs but was terminated due to aggressive behavior and suspicion of stealing. He consistently denies any wrongdoing, making excuses for his actions, and blames others for his failure. When confronted about his behavior, Tom fails to show remorse for having broken the law and physically or emotionally hurting others.
Most recently, Tom was caught breaking and entering an apartment building. His parents have admitted him to the hospital because they realize they can no longer control his behavior and fear that his recent crime will result in a prison sentence. Tom is diagnosed with antisocial personality disorder.

Based on the case study, do the following.
Identify three NANDA-approved actual nursing diagnoses and two risk for nursing diagnoses for the client (

). Actual nursing diagnoses must be a 3-part statement and risk for nursing diagnoses must be a 2-part statement.
1. Based on the #1 priority nursing diagnosis, develop a nursing process using the table provided below
. The table will include:
· Three client goals/outcomes specific to the priority nursing diagnosis (must meet the SMART criteria).
· Six evidence-based nursing interventions specific to the achievement of client goals/outcomes.
· Rationales based on evidence for each nursing intervention. With each rationale, include the source (last name of the author, year of publication, and the page number).
Example:
Intervention: Teach the family how to provide physical care for the client (bathing, feeding, and ADLs). Rationale: Improved self-efficacy regarding how to care for loved ones has been shown to decrease caregiver burden (Gonyea, 2015, p. 41).
2. Include a reference page (must be in APA format, 7th edition).
· For this paper, you will have in-text citations (for your rationales) and a reference page with all your sources.
