Discuss what it means to be an agent of change from the context of that experience.
Change exists daily on the in the health care landscape and the change agent in the medical scenarios have become the historical term as those leading the change assists people inclusive of the professional practitioners and the patients. The major function for nurses is being the agents of change and they have a responsibility to continue looking for ways of impacting change in the system of healthcare. If nurses can expand their roles to become agents of change in the medical field then they stand a chance of playing a critical role in shaping the future of the care system (Rodin & Janis, 1979).
Discuss the circumstances in terms of change theory.
The change theory is related to the nursing process whereby problems which may occur after the discharge are assessed and identified by the nurse to ascertain patient’s satisfaction. The circumstance for this is seen in a situation where the contribution of the nursing in the healthcare reforms demands for the change in understanding the practices and the implementations with the aim to deliver quality. In a situation when the numbers of readmissions turns up to be extremely high would definitely call for action to improve nursing services to the affected patients after the discharge.
Describe any barriers that were resisting change.
Unfortunately, there are barriers which exist in the nursing system that keeps limiting nurses from being effective agents of change. The major barrier that prevents nurses from being the change agent leaders is that they are not allowed to fully practice their training skills and knowledge in the medical organisation (Milner, Estabrooks & Humphrey, 2005). The nurses too are not recognised as the essential functioning discipline in the health care system. These limitations lower the change making process. Having new ways of thinking on the capabilities of the nurses by allowing them to participate in health care decision making can reduce the barriers.
Identify a needed change in the unit or organization. Using the change process, outline the steps you would take to initiate that change.
The scenario which needs the change in the organisations is the readmission of the patients. Patients normally returns back to the hospital just within few weeks after discharge which could be due to doomed caring conditions of the patients. Nurses should facilitate change in the reduction of hospital readmission. As the nurse, would ensure that proper care of the high risky patients is considered after the discharge. I will conduct the evaluation on the conditions of the patient for three months after the patient have been discharged and shows collaboration with the care givers and the family. This will effectively ensure reduction in the readmission of the patients.
Select one of the goals and identify the improvement model used, the standard established by the Joint Commission, the indicator that is used to determine whether that standard is met or not, and the evidence that the indicator is derived from.
The goal of the organisation is to ensure effective healthcare operations for satisfaction of the patient. The indicator for this goal will be measured on the basis to improvement on whether the primary change of the action has led to the desired direction and the belief on the good performance which has got reflection on the effectiveness of the strategy (Davies, Nutley & Mannion, 2000). The public reports and the comments on the performance with regard to the system changes. Interpretation of the data on the patient’s comments may consequently help the organisation to make the critical decisions for more effective quality care.

The quality improvement in the in healthcare is necessary to enhance safety of the patients and the degree of the healthcare services in the organisation( Koeck,1998).The organisations am operating at have been experiencing the problem of the medical errors as a result of the faulty systems and equipment. The organisations are therefore sorting to improve the condition through adoption of the various techniques which aid in improving the process of the system in the organisation by identifying the inefficient and ineffective in the system to initiate change. The assessment on the performance is conducted and analysing the results for the implementation and improving quality.
